All On 4 Plus® is an indication-specific system for the replacement of ALL teeth ON FOUR or more dental implants (depending on individual clinical and anatomicalconditions), where the FINAL teeth are fitted typically within 24 hours.

The surgery involves tilting of the back implants in order to provide adequate supportfor the new teeth using less implants. With less hardware there is also an improved flexibility in the design of the teeth for optimal function, strength and aesthetics.

This aspect is particularly crucial, especially within the initial three months of healing when the implants are at their most delicate. This consideration has often been overlooked in the denture conversion technique (explained further below) due to the absence of viable alternatives. Introduced in 2004 by Dr Alex Fibishenko, Immediate Final Teeth™ on four dental implants is an All On 4 Plus® hallmark, offering a secure alternative to the delicate denture-conversion temporary teeth method. In full-set dental implant treatment, the final prosthesis's robust structure supports prosthetic teeth, linking dental implants rigidly. Additional Implants

Less is more in terms of cleanability and design, and four is ideal and symmetrical. However, in some cases, such as in poor quality bone, or when clenching power is known to be excessive, additional implants may be required to improve system’s ability to withstand the functional pressures. It may also be an option when wanting to increase the number of teeth beyond the typical arch size of 12 teeth. Irrespective of whether additional implants are required, we make every effort to position them correctly for enhanced comfort and spread them optimally to simplify cleaning.

Typically considered in the upper only, bone grafting may be used simultaneously with implant surgery (when required) to rebuild the bone volume in the sinus areas (below left). This facilitates positioning and spreading of the implants and it also improves contingency with more available bone in the future. Grafting may also be used with Zygoma Implants when the structure and/or quality of the jawbone is extremely limited.

The duration of implant surgery hinges on factors such as the condition of existing teeth and jawbone. Typically, the comprehensive process spans one to three hours. Personal experiences differ from person to person but typically pain is not a common complaint. In fact most patients reposrt that they have not taken the pain medication that we prescribed. Most procedures are done under a general anaesthetic, so you are completely asleep, but we still use a local anaesthetic to completely numb your entire jaw to reduce the chances of post operative pain. It's normal to feel some discomfort while you're recovering, but over-the-counter pain medication is usually enough to keep you comfortable.
